Setup
Place the Bristol map in the center of the table. Place the three carts (Birch, Cedar, Oak) on their designated starting spaces. Each player chooses a character card and its matching pawn. All chosen pawns are placed in the pouch, shaken, and then randomly placed into the carts from front to back, according to the player count table. Remove all symptom cards with a value of '4' from the symptom deck. Shuffle the remaining symptom cards and deal two to each player face down. If any player receives two '3' value symptom cards, they reveal them, discard them, and draw two new symptom cards. Shuffle the discarded '3' cards back into the deck. Shuffle the '4' value symptom cards back into the symptom deck and place it face down on the 'New Symptoms' space. Shuffle the remedy cards and deal them to players according to the player count table, placing the remaining cards face down on the 'Remedies' space.

On your turn
The game is played in multiple rounds. In each round, one player rolls all six dice for the entire group. Then, each player, in clockwise order, takes their turn. On your turn, you have three options: 1) Reroll two dice: Take any two dice from the table and reroll them. The other four dice remain unchanged. 2) Draw a remedy card: Draw one remedy card from the deck. You can hold a maximum of three remedy cards. 3) Make a movement: You can perform one of three types of movements: 'Elbow' (move your pawn to the front of your current cart), 'Dash' (move your pawn to the last available spot in the cart ahead of yours), or 'Push' (push a pawn behind you to a cart further back, or push yourself back).
Scoring
At the end of each round, after all players have taken their actions, the carts move. Each cart moves a number of spaces equal to the sum of the dice of its color (rats and apples are summed). If two or more carts land on the same space, the cart that moved first is considered to be in front. If a cart exits the city, all players in that cart reveal their symptom cards. If all players in the exiting cart are healthy (total symptom value less than 6), those players win. If any player in the exiting cart is infected, all players in that cart die and are eliminated from the game.
Game end
The game ends immediately when one of the following conditions is met: 1) A cart successfully escapes the city with all healthy passengers. The players in that cart win. 2) All players are infected with the plague (total symptom value of 6 or more). In this case, everyone loses. 3) All carts are infected, and no healthy cart escapes after an additional round. The last healthy player wins. 4) All players are eliminated from the game (either by being pushed out of the last cart or by being in an infected cart that exits the city).
